  • Please note that all checklists and forms in this packet are required for submission (except the optional forms on pages 13 and 14).

  • Is the Applicant Eligible?

  • The applicant owns and occupies the property as their primary residence. (applicant temporarily displaced due to natural disaster, fire, flooding, or other similar circumstance may be eligible).
  • The applicants's household is at or below 120% AMI (AMI limits by county and household size can be found on the eligility questionnaire on nysmap.org)
  • After NYS-MAP intervention the applicant's front-end debt-to-incom ratio (DTI) is at or below 40% and back-end DTI is at or below 55%. If the applicant's front-end DTI is above 40% but no more than 45%, the applicant must have a budget surplus of at least $300 to be considered eligible.
  • The applicant's household budget does not show a deficit after NYS-MAP intervention.
  • The applicant does not own additional residential real property, defined as atleast 50% ownership interest (applicants who are losing their additional property in foreclosure, or otherwise unable to liquidate additional property, may be eligible for NYS-MAP.)
  • The applicant has not received NYS or NYC-MAP funds previously.

1.Fees

There are no application fees, credit report fess, lock-in fees, title insurance fees, or property appraisal fees charged in connection with a New York State Mortgage Assistance Program Loan (“NYS -MAP Loan”).

2.Interest Rate

The interest rate on all NYS-MAP Loans is 0%. This rate will not change at any time.

3.Early Repaymetn/Prepayment

The early NYS-MAP requires no early repayment prior to maturity except upon sale of the property or violation of NYS-MAP Loan requirements. The NYS-MAP Loans may be prepaid or refinanced at any time prior to maturity without penalty.

4.Baloon Payment

The NYS-MAP Loan will not amortize during its term. Consequently the full amount of NYS-MAP Loan will be payable at maturity. If not repaid or refinanced, you may be loose the property or may be subject to considerable tax liabilities if the loan is restructured or written-down.

5.Refinancing Risk

Refinancing will depend upon, among other things, your future financial situation, the property value and market conditions.

6.Non-Assumption

The NYS-MAP Loans may not be assumed by any other person without express written consents.

7.Adverse Determination

To the extent that it is determined that the applicant is not eligible for NYS-MAP Loan, the applicant will receive a copy of the credit report, if any.

8.Loan Adminstrator

Applicant may contact Sustainable Neighborhoods, LLC (“SN”), the administer of the New York State Mortgage Assistance Program, at any point during the process of application, commitment, and closing of NYS-MAP Loan.

I/we have applied for NYS-MAP Loan from SN. As part of the application process, SN may verify information contained in my/our loan application and in other documents required in connection with the loan, either before the loan is closed or as part of its quality control program.

I/we authorize you to provide to SN any and all information and document that they request. Such information includes, but is not limited to: employment history and income; bank, money market and similar account balances; credit history; and copies of income tax returns. I/we further authorize SN to order a consumer credit report and to verify any other credit information, include past and present mortgage and landlord references.

DODD-Frank Certification
