For the purpose of NYS-MAP, all person on the deed are considered applicants and should submit all required documents. All submitted documents must be current (dated within 90 days).

There are no application fees, credit report fess, lock-in fees, title insurance fees, or property appraisal fees charged in connection with a New York State Mortgage Assistance Program Loan (“NYS -MAP Loan”).
The interest rate on all NYS-MAP Loans is 0%. This rate will not change at any time.
The early NYS-MAP requires no early repayment prior to maturity except upon sale of the property or violation of NYS-MAP Loan requirements. The NYS-MAP Loans may be prepaid or refinanced at any time prior to maturity without penalty.
The NYS-MAP Loan will not amortize during its term. Consequently the full amount of NYS-MAP Loan will be payable at maturity. If not repaid or refinanced, you may be loose the property or may be subject to considerable tax liabilities if the loan is restructured or written-down.
Refinancing will depend upon, among other things, your future financial situation, the property value and market conditions.
The NYS-MAP Loans may not be assumed by any other person without express written consents.
To the extent that it is determined that the applicant is not eligible for NYS-MAP Loan, the applicant will receive a copy of the credit report, if any.
Applicant may contact Sustainable Neighborhoods, LLC (“SN”), the administer of the New York State Mortgage Assistance Program, at any point during the process of application, commitment, and closing of NYS-MAP Loan.

I/we have applied for NYS-MAP Loan from SN. As part of the application process, SN may verify information contained in my/our loan application and in other documents required in connection with the loan, either before the loan is closed or as part of its quality control program.
I/we authorize you to provide to SN any and all information and document that they request. Such information includes, but is not limited to: employment history and income; bank, money market and similar account balances; credit history; and copies of income tax returns. I/we further authorize SN to order a consumer credit report and to verify any other credit information, include past and present mortgage and landlord references.
It is understood that the copy of this form will also serve as authorization of the foregoing.

The following information is required by the Federal government in accordance with the Dodd-Frank Wall Street Reform and Consumer Protection Act (Pub.L.111.203). You are required to furnish this information. The law provides that no person shall be eligible to receive assistance from this mortgage assistance program if such person, in connection with a mortgage or real estate transaction, has been convicted within the last 10 years, of any one of the following: (A) felony, larceny, theft, fraud or forgery, (B) money laundering or (C) tax evasion.
In making this certification, I/we certify under penalty of perjury that all of the information in this document is truthful and that I/we understand that the lender, the U.S Department of the Treasury, or their agents may investigate the accuracy of my/our statements by performing routine background checks, including automated searches of federal, state and country databases to confirm that I/we have not been convicted of such crimes. I/we also understand that knowingly submitting false information may violate Federal law.
The following information will not be used to determine your eligibility for the New York State Mortgage Assistance program. It is used solely for tracking purposes.
The following information is requested by the Federal Government for certain types of loans related to the dwelling in order to monitor the lender’s compliance with equal credit opportunity, fair housing and home mortgage disclosure laws. You are not required to furnish this information, but are encouraged to do so.
The law provides that a lender may not discriminate either on the basis of the information or on whether you choose to furnish it. A contributor is any individual that that regularly contributes to household income but is not listed on the property deed. A contributor can be a household resident or a non-resident. Resident contributors must submit proof of income and proof of residency, while non-resident contribution income must be verified by 6 months proof of deposits in the applicant’s bank statement.
